Reablement Care Durham
What Is Reablement Care?
Reablement care is a short-term, intensive form of support designed to help individuals regain the skills, confidence, and ability to manage daily life independently — following a period of illness, injury, surgery, or hospital admission.
Unlike standard home care — which focuses on doing tasks for a person — reablement care is centred on helping people do things for themselves again. Our carers work alongside individuals, encouraging and supporting them to rebuild their capabilities step by step, at a pace that is right for them.
Reablement is not about dependency. It is about empowerment. Our aim is always to reduce the level of support needed over time — celebrating each milestone and helping individuals regain as much independence as possible in their own home.
At Almond Flex Care, our reablement service is delivered by trained, compassionate carers working in close collaboration with GPs, occupational therapists, physiotherapists, district nurses, and other healthcare professionals — ensuring a truly joined-up approach to your recovery.
Understanding the Difference
Reablement Care vs Standard Home Care
Both reablement and standard home care are delivered in your own home — but they have very different goals. Understanding the difference helps families choose the right support at the right time.
| Reablement Care | Standard Home Care | |
|---|---|---|
| Primary goal | Rebuilding independence and capability | Providing ongoing daily assistance |
| Duration | Typically short-term (weeks to months) | Can be long-term or ongoing |
| Approach | Encouraging the person to do tasks themselves | Carer performs tasks on behalf of the person |
| Focus | Recovery, rehabilitation, and skill-building | Maintenance, safety, and daily support |
| Outcome | Reduce reliance on care over time | Maintain a consistent level of support |
| Best suited to | Post-discharge, post-surgery, post-illness recovery | Long-term conditions, age-related decline |
“Reablement is about possibility — rebuilding what has been lost and discovering what can still be achieved.”
Who We Support
Who Benefits from Reablement Care?
Reablement care is suitable for a wide range of individuals — any person who has experienced a setback and wishes to rebuild their independence and ability to manage at home.
Post-Hospital Discharge
Individuals recently discharged from hospital who need structured support to recover safely at home and avoid readmission.
Post-Surgery Recovery
People recovering from planned or emergency surgery who require short-term intensive support whilst healing and regaining mobility.
After a Fall or Injury
Individuals who have experienced a fall, fracture, or injury and need support rebuilding their confidence and physical capability.
Stroke Recovery
People recovering from a stroke who are working to rebuild motor skills, speech, confidence, and independence at home.
Following Serious Illness
Those recovering from serious or prolonged illness — including respiratory conditions, infections, or cancer treatment — who need support regaining strength and daily living skills.
Individuals at Risk of Long-Term Dependency
People who, without structured reablement support, may become unnecessarily reliant on ongoing home care — and who could regain independence with the right intervention.
Our Approach

Our reablement service is structured, purposeful, and always centred on your individual recovery goals. Here is what you can expect from the very first contact.
Referral or Self-Referral
Contact us directly, or ask your GP, hospital discharge team, occupational therapist, or social worker to refer you to our service. We respond to all referrals promptly and with care.
Free Assessment & Goal Setting
A trained care coordinator visits you at home to assess your current abilities, understand your recovery goals, and design a reablement plan tailored specifically to you — in collaboration with your wider healthcare team.
Structured Reablement Support
Your dedicated reablement carer begins working with you at home — supporting and encouraging you through daily living tasks, exercises, and activities designed to rebuild your independence step by step.
Regular Reviews & Reduction of Support
We review your progress regularly — celebrating achievements, adjusting goals, and gradually reducing the level of support as your independence grows. Our aim is always for you to need us less, not more.
