Reablement care is a short-term, intensive form of support designed to help individuals regain the skills, confidence, and ability to manage daily life independently — following a period of illness, injury, surgery, or hospital admission.

Unlike standard home care — which focuses on doing tasks for a person — reablement care is centred on helping people do things for themselves again. Our carers work alongside individuals, encouraging and supporting them to rebuild their capabilities step by step, at a pace that is right for them.

Reablement is not about dependency. It is about empowerment. Our aim is always to reduce the level of support needed over time — celebrating each milestone and helping individuals regain as much independence as possible in their own home.

How Our Reablement Care Service Works

Our reablement service is structured, purposeful, and always centred on your individual recovery goals. Here is what you can expect from the very first contact.

Referral or Self-Referral

Contact us directly, or ask your GP, hospital discharge team, occupational therapist, or social worker to refer you to our service. We respond to all referrals promptly and with care.

Free Assessment & Goal Setting

A trained care coordinator visits you at home to assess your current abilities, understand your recovery goals, and design a reablement plan tailored specifically to you — in collaboration with your wider healthcare team.

Structured Reablement Support

Your dedicated reablement carer begins working with you at home — supporting and encouraging you through daily living tasks, exercises, and activities designed to rebuild your independence step by step.

Regular Reviews & Reduction of Support

We review your progress regularly — celebrating achievements, adjusting goals, and gradually reducing the level of support as your independence grows. Our aim is always for you to need us less, not more.
